Admissions


Patient Registration/Admissions - (440) 816-8600  

Patients who need to be admitted to Southwest General for care, should call the Patient Registration/Admissions Department at the number listed above. Staff members from this department will obtain necessary information including, but not limited to:

  • Name
  • Address
  • Insurance Information
  • Diagnosis

Patients will be informed of the date and time to arrive at the health center and will be given directions for where to go in the health center.

 

Patients should check with their insurance provider to determine insurance coverage.

 

Admission, Scheduling and Discharge

Southwest General strives to make the admissions and scheduling process as easy as possible for patients who may need an inpatient stay or outpatient testing. It is recommended that patients check with their insurance provider to determine insurance coverage prior to calling the Health Center. Southwest General participates in most major insurance plans. Our philosophy is to create sensible partnerships with the insurance plans covering members of our communities. However, patients may find that their insurance coverage requires that certain services be received at specific locations.
 
Do You Need to Schedule Testing?
Patients who need to schedule outpatient testing may do so through the Central Scheduling/One Call Department. Staff members from this department will obtain necessary information from you, schedule your testing and provide instructions regarding your tests and where to go for those tests. Patients also will receive instructions for any necessary preparation prior to testing. For more information, call 440-816-8605.

Patients who need to be admitted to Southwest General for care should call the

Discharge
To make sure your return home goes as smoothly as possible, your physician and nurses begin planning your discharge shortly after admission. We make every effort to discharge our patients by 11 a.m. However, circumstances beyond our control may delay your discharge until later in the day. Discharge is determined by your physician. Your physician or nurse will provide instructions for your continued care at home and will let you know how to schedule follow-up appointments with your physician or a Health Center service if needed.
