Performed by a team of professionals specializing in the care of older adults, the Elder Evaluation program is an outpatient geriatric assessment. Led by a board-certified geriatrician, the assessment takes an indepth look at the individual's physical and physiologic needs, working with the family doctor to obtain baseline information. Over the course of three to four weeks, we spend time with the patient both in the office and in their home determining their needs. Our findings and recommendations are presented in a summary conference with the patient and family.
