HOW CAN I DONATE?

There are times when a person wishes to make a donation to support the care Hospice Services provides to the community.  The following are funds from which to choose.

The Thomas P. Perciak Family Residential Hospice
These donations are designated exclusively to provide comfort and care for the patients at the residential facility in Strongsville, Ohio.  Donations to this fund also are used for unique, special needs within the building.
 

Community Care Fund in Memory of Dennis C. Simmerman

This fund allows Hospice Services to reach out to those in the community with financial needs.  These needs may include anyting from a short-term stay at the residential facility to providing transportaion for an out-of-town family memebr to visit the patient, paying utility bills, purchasing food and supplies when the patient does not have the funds to do so.  An evaluation is conducted to identify the needs of our patients.

Nurse Education
This fund provides continuing education to nursing staff and hospice aides.  These donations are used to offset the expense of the Hospice Nurse Certification program as well as other training opportunities.  This continuing education empowers the Hospice Staff to learn about new developments in patient care.  Educational topics include: pain and symptom management; coping with the person with dementia; identifying the unique needs of the terminally ill person; identifying and supporting the needs of the family; and facilitating a life review.  The education fund is vital to providing the best professional care to our patients while offering the intimate care of a similar hospice program.

General Fund

This fund is used for the overall needs of the Hospice Services program:

  • To purchase specialized items that are not commonly included in a budget, such as the television in the Community Room at the residential facility.
  • To grant requests as designated by the family or patient and approved by Hospice Services administration.
  • To maintain and beautify the grounds at the residential facility, including the fountain, gardens and courtyard.
  • To provide other equipment, supplies and services as needed for the care of patients enrolled in the Hospice Services program.

GIVING METHODS

To make a donation to Hospices Services, you may write a check or use a credit card by completing the donation form or calling the Southwest Community Foundation at 440-816-6713.

Employer Matching Gifts
Would you like to double your donation to Hospice Services?  If so, ask your employer if they match charitable contributions made by their employees.  All you need to do is request a matching gift form, complete it, and send it with your donation.  We will do the rest!

Memorial Gifts

A family may designate Hospice Services of Southwest General to be the recipient of donations made in lieu of flowers through a funeral home.  Your local funerak home director can help you with this process.  Envelopes for this also can be provided to you by Hospice Services staff.

Estate Giving

Sometimes a person wants to make a difference but does not have the financial ability to do so.  For this person, one method of making a difference is through estate giving.  Hospice Services can be a designated beneficiary of life insurance, IRAs, stocks or in a will.  If you are interested in this method of supporting Hospice Services, please contact Bobbie Simmerman, executive director of Southwest Community Health Foundation, at 440-816-6711.

Wish List

There are times when a person would like to purchase a specific item to help with the Hospice Services program.  The following are items on the Hospice Services Wish List:

  • nursing education
  • music therapy for patient 
  • art therapy for patient and his/her children
  • desktop computer
  • bereavement books for children
  • small wicker baskets 
  • travel-sized soap, shampoo and lotions for guests
  • furniture for the Great Room
  • personal amplifiers for hard-of-hearing patient
