Southwest General’s Neighborhood Care Center is focused on helping to strengthen the quality of life in area communities by providing primary health care services at no cost for qualified residents of the Health Center's taxing district -- Berea, Brook Park, Columbia Township, Middleburg Heights, Olmsted Falls and Strongsville.
Criteria Designed to Assure Care for Those Who Need It
Those eligible to be considered to receive primary health care at no cost through Southwest General’s Neighborhood Care Center must:

Have no health care insurance coverage and/or no eligibility for coverage

Not be receiving government medical care assistance or aid

Have an income level within 200% of the federal poverty level

Live within one of Southwest General’s six taxing districts (Berea, Brook Park, Columbia Township, Middleburg heights, Olmsted Falls and Strongsville)

Complete a written application
Primary Care Provided By Dedicated Professionals
At the Neighborhood Care Center, confidential and compassionate treatment is provided by qualified physicians, nurses and clerical staff members who volunteer their time. Services include physical exams and treatment covering a full range of basic medical problems.
Patients eligible for service are seen by appointment on Tuesday evenings from 6 to 8 p.m. and two Thursday evenings per month from 6 to 8 p.m. Prescriptions are available at a reduced cost through the Southwest Community Pharmacy, a retail pharmacy located in Southwest General’s Main Lobby.
