Residential Hospice

Residential Hospice
When patient needs exceed the care that can be provided by family and friends

The Thomas P. Perciak Family Residential Hospice of Southwest General offers a comfortable setting staffed by professional hospice personnel ready to provide compassionate and comprehensive inpatient care in a home-like environment.

Levels of Care

Symptom Management

This level of care involves frequent nursing assessment and care planning to address symptoms that cannot be easily managed at home or in another setting. This is typically a short-term stay in our residential center with a goal of returning home or to another facility. The hospice attending physician is involved in determining this level of care’s duration.

Respite Care

Respite care services are provided at our residential center to allow the caregiver a time to rest or attend to personal matters, knowing that the patient is being cared for by trained staff members. To inquire about the use of respite care, please talk with your hospice nurse or social worker.

Residential Care

This level of care is intended for short-term use with the acknowledgement that each patient’s needs are individually assessed and the patient’s length of stay is evaluated regularly by our interdisciplinary team. Room and board is not covered under the hospice Medicare benefit however, all or part of this cost may be covered by private insurers. Our hospice social worker is ready to assist you in helping to make the appropriate financial assessments.
