Donations and Wish List

Donate

Community support continues to help make hospice at Southwest General a valued resource.

There are times when a person wishes to make a donation to support the care Hospice Services provides to the community. The following are areas from which to choose where your generosity will do the most good:

The Thomas P. Perciak Family Residential Hospice

These donations are designated exclusively to provide comfort and care for the patients at the residential facility in Strongsville, Ohio. Donations to this fund also are used for unique and special needs within the building.

Community Care Fund in Memory of Dennis C. Simmerman

This fund allows Hospice Services to reach out to those in the community with financial needs. These needs may include anything from a short-term stay at our residential facility to providing transportation for an out-of-town family member to visit the patient.  It is also used for paying utility bills and purchasing food and supplies when the patient does not have the funds to do so. To assure that the funds are properly used, an evaluation is conducted to identify the needs of our patients.

Nurse Education Fund

This fund provides continuing education to nursing staff and hospice aides. These donations are used to offset the expense of the Hospice Nurse Certification program as well as other training opportunities.  Continuing education of this type empowers the Hospice Staff to learn about new developments in patient care. Educational topics range from pain and symptom management to facilitating a life review. This education fund is vital to providing the best professional care to our patients while offering the intimate care of a similar hospice program.

General Fund

This fund is used for the overall needs of the Hospice Services program and includes:

  • Purchasing specialized items that are not commonly included in a budget, such as the television in the Community Room at the residential facility.
  • Granting requests as designated by the family or patient and approved by Hospice Services administration.
  • Maintaining and beautifying the grounds at the residential facility, including the fountain, gardens and courtyard.
  • Providing other equipment, supplies and services as needed for the care of patients enrolled in the Hospice Services program.

Giving Methods

  • Donations. To make a donation to Hospices Services, you may write a check or use a credit card by completing the donation form or calling the Southwest Community Foundation at 440-816-6713.
  • Employer Matching Gifts. If you would you like to double your donation to Hospice Services, ask your employer if they match charitable contributions made by their employees. All you need to do is request a matching gift form, complete it, and send it with your donation. We will do the rest.
  • Memorial Gifts. A family may designate Hospice Services of Southwest General to be the recipient of donations made in lieu of flowers through a funeral home. Your local funeral home director can help you with this process. Envelopes for this also can be provided to you by Hospice Services staff.
  • Estate Giving. Occasionally a person would like to help make a difference but does not have the financial ability to do so. For this person, one method of making a difference is through estate giving. Hospice Services can be a designated beneficiary of life insurance, IRAs, stocks or in a will. If you are interested in this method of supporting Hospice Services, please contact Bobbie Simmerman, executive director of Southwest Community Health Foundation, at 440-816-6711.
  • Wish List. There are times when a person would like to purchase a specific item to help with the Hospice Services program. The following are items on the Hospice Services Wish List:
    • Nursing education
    • Music therapy for patient
    • Art therapy for patient and his/her children
