Setting Up Your MyHealth Account
If you have received care from Southwest General, you may self-enroll (register)
for a Southwest General MyHealth account.
When registering, you will be asked to provide the following information:
- First name
- Last name
- Date of birth
- Southwest General medical record number (MRN)
To find your MRN:
- Check your discharge instructions after a hospital stay or ER visit
- Look on your billing statements
- Ask your Southwest General caregiver at your next visit
- Visit Southwest General’s Medical Records Department at 18697 Bagley
Rd., Middleburg Heights, Monday–Friday, 8:30 a.m.–4p.m., excluding
holidays. For security purposes, medical record numbers are not provided by phone.

If you have problems setting up your account, call Technical Support for Southwest General’s MyHealth at 1-877-621-8014,
24-hours a day, seven days a week.
Some individuals, including those seeking proxy access to a loved one’s
health information, will not be able to self-enroll. Instead, they will
need to be sent an email invitation to register for Southwest General’s
MyHealth. To receive an invitation, ask your caregiver at your next Southwest
General appointment or visit the Medical Records Department on Southwest
General’s Main Campus, 18697 Bagley Rd., Middleburg Heights, Monday-Friday,
8:30 a.m.-4:30 p.m.
If you receive an email invitation to register for Southwest General’s
MyHealth, open the invitation, click on the link inside to accept the
invitation and follow the on-screen instructions to set up your account.
