Medical Records

Southwest General provides patients with access to their medical records in various ways. Additionally, patients and their Personal Representatives have the right to review and request amendments to their medical information. For more information regarding patients’ rights under the HIPAA laws, refer to our Notice of Privacy Practices webpage.

Accessing Medical Records

HealtheLife patient portal

  • The HealtheLife patient portal provides individuals with secure online access to portions of the electronic medical record 24/7/365. Creating an account is free and takes only a few minutes.

Electronic Request (eRequest) for Records

All Southwest General records, including hospital records and SGMG Physician Office records, can now be easily requested using this link. This link goes to a secure external site hosted by Datavant, our release of information partner, and can be used to request records for various purposes. The website will confirm the identity of the requester and send a copy of the completed request via secure email. This site is compatible with most computers and handheld devices (e.g., smart phones, tablets), is simple to use, and allows the requester to submit one request for all Southwest General records!

Requesters such as the patient, legal guardian, or attorney may use this eRequest option.

Using eRequest allows various methods of how to receive the requested records –

  1. Secure electronic copy via email;
  2. Paper copies mailed to the address provided; or
  3. Paper copies picked up in the Medical Records Department of the hospital.

Paper Request for Records

If the eRequest option is not for you, you may print this form and submit your request for hospital records or SGMG Physician Office records in writing directly to the Medical Records Department at Southwest General Health Center or to the SGMG Physician Office where services were received. The form may be emailed to swroi@swgeneral.com or mailed/dropped off at the address at the bottom of the form. For questions, please call Medical Records at 440-816-8480.

Radiology Images Only

Although requests for radiology records may be submitted using the eRequest option above, you may also submit this form for radiology images and reports only. Using a computer, the form can be printed and completed manually. The form may also be completed electronically by 1) using Adobe Acrobat Reader or 2) using a web browser to open the form, complete it and save it to your computer. Once saved to your computer, the completed form can be emailed to imagesharing@swgeneral.com or mailed/dropped off at the address at the bottom of the form. Request an Amendment to Medical Records

Patients have a right to request changes, or an amendment, to their medical record when they believe the information may be inaccurate as written. For convenience, the following form may be used to submit a request to amend the medical record. Southwest is not obligated to make all requested changes however careful consideration will given to each request. All requests must be in writing (signed and dated).
