Thomas P. Perciak Family Residential Hospice

About This Location

There are times when the needs of the hospice patient exceed the care that can be provided by family and friends. The Thomas P. Perciak Family Residential Hospice of Southwest General offers a comfortable setting and is staffed with professional hospice personnel who can provide for inpatient care in a home-like environment.

Levels of Care

Symptom Management:
This level of care involves frequent nursing assessment and care planning to address symptoms that cannot be easily managed at home or in another setting. This is typically a short-term stay in the residential center with a goal of returning to home or other facility. The hospice attending physician is involved in determining the duration of this level of care.

Respite Care:
Respite care services are provided at the residential center to allow the caregiver a time to rest or attend to personal matters, knowing the patient is being cared for by trained staff members. To inquire about the use of respite care, please talk with your hospice nurse or social worker.

Residential Care:
This level of care is intended for short-term use with the acknowledgement that each patient’s needs are individually assessed. Length of stay is evaluated regularly by the interdisciplinary team. Room and board is not covered under the hospice Medicare benefit. All or part of this cost may be covered by private insurers. The hospice social worker is available to assist with financial assessment.

Amenities

The Thomas P. Perciak Family Residential Hospice offers patients and families a variety of amenities. Each room is a private room that includes a private bath, patio and accommodations for families to stay overnight—even pets are welcome to visit. Additionally, food service is available for patient and guests. Visitors

Visitors may visit 24 hours a day as long as that meets the patient’s wishes. Visitors must enter through the front vestibule at all times. The doors are locked at 8 p.m., so please ring the doorbell when arriving after that hour. Children are welcomed visitors at the residential center. Our community room has age-appropriate toys, games and reading materials. For the safety of all children, we ask that an adult accompany children at all times.
