Medical Records

Southwest General provides patients with access to their medical records in various ways. Additionally, patients and their Personal Representatives have the right to review and request amendments to their medical information. For more information regarding patients’ rights under the HIPAA laws, refer to our Notice of Privacy Practices webpage.

Accessing Medical Records

Set up a Southwest General HealtheLife patient portal account

The HealtheLife patient portal provides online access to portions of the electronic medical record 24/7/365. Setting up an account is free and takes only a few minutes.

Request Hospital Medical Records

Submit a written request to Southwest General’s Health Information Management Department by completing the Authorization to Release Health Information form.

Access the Authorization to Release Health Information form here.

Complete the form and submit it using one of the following options:

  1. If you completed the form using Adobe Reader, click the SUBMIT button at the bottom of the form. It will automatically open your email and attach the form. Just click send to submit.
  2. If you completed the form in a web browser, right click on the form and save it to your computer. Email the completed form to Southwest Release of Information - [email protected]
  3. Print out the completed form and mail it to:

Request Physician Office Medical Records

Download, complete and print the Authorization to Release Health Information form. Bring it directly to the Southwest General Physician Office where services were received.

Request an Amendment to Medical Records

Patients have a right to request changes, or an amendment, to their medical record. For convenience, the following form may be used to submit a request to amend the medical record. Southwest is not obligated to make all requested changes however careful consideration will given to each request. All requests must be in writing (signed and dated).
