Medical Records

Southwest General provides patients with access to their medical records in various ways. Additionally, patients and their Personal Representatives have the right to review and request amendments to their medical information. For more information regarding patients’ rights under the HIPAA laws, refer to our Notice of Privacy Practices webpage.

Accessing Medical Records

HealtheLife patient portal

  • The HealtheLife patient portal provides individuals with secure online access to portions of the electronic medical record 24/7/365. Creating an account is free and takes only a few minutes.

Electronic Request (NEW!)

All Southwest General records can now be easily requested using this link. Clicking the link will take you to a secure external site hosted by Datavant, our release of information partner, where you can request records for various purposes, confirm your identity, and even receive a copy of your completed request via secure email. This site is compatible with most computers and handheld devices, is simple to use, and in many cases will get records where they are needed in less time!

Hospital Medical Records

Hospital visit or stay records may be submitted in writing directly to the Medical Records Department at Southwest General Health Center using this form. The form may be emailed to swroi@swgeneral.com or mailed/dropped off at the address at the bottom of the form. For questions, please call Medical Records at 440-816-8480.

SGMG Physician Office Medical Records

To request medical records from a Southwest General Medical Group (SGMG) physician office, print the Authorization Form and send the completed form to the Southwest General physician office where services were received. Call the physician’s office to obtain contact information for sending your request.

Radiology Images

To request radiology images, complete this form. The form can be printed and completed manually or completed electronically by 1) using Adobe Acrobat Reader or 2) opening in a web browser and completed, then saved to your computer. The completed form can be emailed to imagesharing@swgeneral.com or mailed/dropped off at the address at the bottom of the form. For questions, please contact Radiology at 440-816-8770

Request an Amendment to Medical Records

Patients have a right to request changes, or an amendment, to their medical record when they believe the information may be inaccurate as written. For convenience, the following form may be used to submit a request to amend the medical record. Southwest is not obligated to make all requested changes however careful consideration will given to each request. All requests must be in writing (signed and dated).
