Southwest Community Health Foundation
Supporting Southwest General through Charitable Giving
Southwest General began as a small community hospital in 1920, founded
by residents of the surrounding communities after a devastating flu epidemic.
What the residents of these communities hoped to establish was a local
source for health care, keeping patients close to home. Nearly 100 years
later, Southwest General not only meets the need for health care close
to home but also does so with an array of award-winning programs and services.
To support ongoing growth and development of hospital services, programs
and facilities, Southwest Community Health Foundation was created in 1985
and became fully operational in 1986.
The Foundation helps to support programs and services for patients in the
Greater Cleveland area, including northern Medina, eastern Lorain and
southwestern Cuyahoga counties. Leadership at the Foundation includes
a committed Board of Trustees and a talented team of non-profit professionals,
supported by the work of our dedicated volunteer committees. Your generous
support through giving enables us to continue serving our community.
Every gift to The Foundation is important, regardless of the amount. Below
are ways that you can help support our charitable efforts. We are truly
grateful for your support and encourage you to contact the Foundation
offices at any time should you have questions.
There are many ways to support The Foundation’s work:
Cash Gifts - A gift of cash, most typically made through a personal check, is one
of the easiest ways to support The Southwest Community Health Foundation.
Gifts along with any special intent (i.e. memorial, honorarium, annual

Online/Credit Card Gifts – The Foundation is glad to accept credit card / online gifts using
Visa, American Express, Discover and MasterCard.

Grateful Patient Gifts – Many patients, or loved ones of a patient, want to honor a special
member of Southwest General’s staff or simply make a gift in recognition
of the exceptional care they received. Find out more on our "Grateful

Planned Gifts – To continue providing quality care to patients of Southwest General,
we ask you to please consider including a gift to us in your overall estate
and financial plans. These types of charitable arrangements let you support
our mission far into the future, while also providing tax and other benefits to you.
Tribute Gifts – Tribute gifts are a wonderful way to honor or memorialize a special
person or occasion in your life by making a gift to The Southwest Community
Health Foundation in the name of a loved one or an important event.
Employee Matching Gifts –Many corporations will match employees’ gifts to the charity
of their choice. Some companies may double or triple the amount of your
gift. It's easy to do:
- Ask your employer or your spouse's employer if they match gifts to
Southwest General or the Southwest Community Health Foundation.
- Obtain and complete the matching gift form. It is usually available from
the Human Resources department or your employer’s website.
- Send the matching gift form along with your gift to the Foundation offices.
- The Foundation completes the necessary matching gift form and will contact
you when the gift is received.
Sponsorship – Sponsorship opportunities for corporations and individuals are
available through our annual golf outing and bi-annual Tiara Gala. Sponsorship
packages range from $1,000-$25,000 plus. For more information about our

Recurring Gift – A recurring gift is an easy way to extend the impact of your gift
by establishing a monthly giving plan where a predetermined gift amount
is automatically submitted on a recurring basis.
