At Southwest General’s Coumadin Clinic, we offer a safe, welcoming, and streamlined approach to managing outpatient warfarin (Coumadin®, Jantoven®) therapy.
Warfarin—also known by its brand names Coumadin® and Jantoven®—is an anticoagulant used to treat and prevent blood clots in individuals with conditions such as clotting disorders, atrial fibrillation, stroke, or mechanical heart valves. Because warfarin (Coumadin®, Jantoven®) requires precise dosing to be both effective and safe, regular and careful monitoring is essential to minimize risks and ensure optimal outcomes.
During each visit, you’ll meet one-on-one with a pharmacist who specializes in warfarin (Coumadin®, Jantoven®) management. They will work directly with you to monitor your therapy, adjust dosages as needed, and help you stay on track with your treatment plan. Additionally, our dedicated team collaborates closely with your healthcare provider to ensure you receive the appropriate follow-up care tailored to your needs.
A referral form needs to be completed by the healthcare provider who is currently managing your anticoagulation therapy. Your provider may contact our office by calling 440-816-6743, and a form can be faxed directly to their office. Once the referral form is received, you will be contacted by a representative of the Coumadin Clinic to schedule your first appointment.
Your first appointment will last approximately 40 minutes. For this appointment, you will need to bring a complete list of all your medications and supplements, which will be reviewed by the pharmacist. Also, you will be asked to complete and provide any relevant medical information. Educational material regarding warfarin (Coumadin®, Jantoven®) will be provided and all questions you may have regarding the medication will be answered.
Follow-up appointments last approximately 10–20 minutes. During each visit, an INR blood test will be done, and results will be immediately discussed with you. If your results are out of range, you and the pharmacist will discuss the possible reasons why your INR is out of range and medication adjustments will be made. The frequency of follow up appointments is specifically tailored to each patient. In the beginning, more frequent visits are required as a safety precaution. When warfarin (Coumadin®, Jantoven®) dosing is stabilized, appointments are usually scheduled once a month.
