Memorial

Flowers with heart in middle with text In Loving MemorySouthwest General's Hospice Services provides terminally ill patients with the support and resources needed to continue an alert, pain-free life and to manage other symptoms so that their last days may be spent with dignity and quality, surrounded by their loved ones.

In 2003 Southwest General opened The Thomas P. Perciak Family Residential Hospice in Strongsville. Our residential facility offers state-of-the-art, 24-hour medical care focused on symptom relief as well as support for patients’ families. However, we understand that everyone’s situation is not the same. We also have dedicated team of in-home hospice caregivers so that patients can remain in their homes and still receive the essential care they need.

Through a memorial gift to our Hospice Services or The Thomas P. Perciak Family Residential Hospice facility, you can pay tribute to a loved one or friend, as well as help future patients and families during the most difficult of times.
