Supporting Patients Post-Hospitalization
Most people look forward to getting out of the hospital, but even after
their initial (acute) care has been completed, they may not be well enough
to return home. For patients who require additional care when discharged
from the hospital, Southwest General offers Post-Acute Services, including:
The goal of this program is to provide a bridge for patients from inpatient
hospital care until they can be released to a skilled nursing facility
or home. Patients in Acute Inpatient Rehabilitation are required to have
a minimum of three hours of combined physical, occupational and speech
therapy per day, meet certain diagnostic criteria, and demonstrate consistent
The Skilled Nursing Facility (SNF) Collaborative is a partnership between
Southwest General and specific skilled nursing facilities in the community.
The goal of the partnership is to improve communication and better coordinate
the care that patients receive from all care providers. When deciding
where to go after being discharged from the hospital, patients needing
skilled nursing care are asked to consider Southwest General’s SNF
Collaborative partners to receive a higher degree of care coordination
and a better overall experience.
