Monday through Thursday 9:00am to 4:30pm and Friday 9:00am to 4:00pm

We require 24-hour notice of cancellations so that we can use the time
to accommodate other patients. You should bring your current insurance
card and a photo ID with you to all appointments.

Prescriptions and Refills
If you need to renew a prescription, you or your pharmacist may call our
office. Pharmacists also may fax a refill request to the office. Although
same-day renewals are available, we request at least one-day’s notice
for prescription refills so that we can be sure to accommodate the needs
of our patients.
The following information will be needed for the prescription request/refill:
- Name of medication
- Strength of medication
- How often medication is to be taken
- Quantity needed (30 day or 90 day)
- Date on which the prescription was last filled
- Pharmacy phone number
- Pharmacy fax number (if you are requesting that the Rx be faxed into pharmacy)
- Phone number where the patient can be reached, in case we need to contact
the patient that day
Once your physician has received and reviewed your test results, we will
contact you by phone with the results and with any further directions/suggestions
from your physician. Our goal is to get test results to you within one
week if possible. If you have questions about your test results, please
contact our office so that the doctor and/or staff can answer your questions.
