Southwest General Medical Group, Inc. - Behavioral Health provides assessment, counseling, crisis intervention and self-help services. Our experienced, board-certified psychiatrists work with adult and geriatric patients to help them deal with mental health or chemical dependency issues and life-changing events (such as the loss of a loved one or divorce).

Welcome to our practice. As a new patient, there are forms that need to be filled out to assist our office staff in making sure that we have the correct information necessary to bill your insurance and provide you with quality care. We have included these forms at the bottom of this page.
Please fill out all the forms completely. The day of your appointment please bring your completed forms, driver’s license or another form of a picture ID, your insurance card and your copy. We collect copay’s at each visit. If you do not have insurance you will be expected to pay at the time of service. We accept check, cash or credit card.
We will call you the day prior to your appointment to remind you of your appointment. If you need to cancel or rescheduled we need a 48 hour notice.
