Medical Records

Southwest General provides patients with access to their medical records in various ways. Additionally, patients and their Personal Representatives have the right to review and request amendments to their medical information. For more information regarding patients’ rights under the HIPAA laws, refer to our Notice of Privacy Practices webpage.

Accessing Medical Records

HealtheLife patient portal

  • The HealtheLife patient portal provides individuals with secure online access to portions of the electronic medical record 24/7/365. Creating an account is free and takes only a few minutes.
  • Create your HealtheLife account here.

HOSPITAL Medical Records

To request medical records for a hospital visit or hospital stay, submit a written request to Southwest General Health Information Management using one of the following options.

IMPORTANT NOTE:

  • This form can be filled out on your computer and can easily be submitted with the click of a button. This requires the form to be downloaded to your computer and opened in Adobe Acrobat Reader.
  • This form can still be filled out on your computer if opened in a web browser, but the submit button will not work. You can then save it to your computer and manually attach and send in an email to SWROI@swgeneral.com, or
  • Print and mail above completed form to:

SGMG PHYSICIAN OFFICE Medical Records

To request medical records from a Southwest General Medical Group (SGMG) physician office, print the Authorization Form and send the completed form to the Southwest General physician office where services were received. Call the physician’s office to obtain contact information for sending your request.

RADIOLOGY RECORDS

To request radiology records, complete the online Radiology Authorization form by clicking here: Radiology Images Release Form. Southwest General will send a link to your email address that will allow you to securely access and download your images from a cloud-based system.

Request an Amendment to Medical Records

Patients have a right to request changes, or an amendment, to their medical record when they believe the information may be inaccurate as written. For convenience, the following form may be used to submit a request to amend the medical record. Southwest is not obligated to make all requested changes however careful consideration will given to each request. All requests must be in writing (signed and dated).
